New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photographic ID
You will need to complete a registration form (GMS1) and new patient health questionnaire (both available from the practice - please speak to our reception team) which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.

NEW PATIENT ONLINE REGISTRATIONS
Our practice is now accepting online registration and this is open to all residents within the borough. To register with a GP practice visit the website:

On the website you will be asked to enter the postcode and based on the area you live you will get a list of GP practices available to choose from. Please select a GP practice, where you will be asked to fill a form and submit your personal information so that we can register you. Your registration will be processed within two working days and you will be contacted by the GP practice about the next steps of registration.
We also welcome in-practice registration and will be happy to help you if you are unable to register online. Please check that your postcode is within our catchment area by visiting the above website.
